Patients with moderate to severe COPD (GOLD stages 2, 3, and 4) that qualify for pulmonary rehab under Medicare guidelines. In addition to COPD, we also accept patients with impairment due to other chronic lung conditions such as interstitial lung disease and pulmonary hypertension as recommended by the American Thoracic Society. Patients must also be physically able to participate and engage in an exercise program to qualify for our pulmonary rehab at home program.

We accept patients with traditional Medicare insurance coverage, Medicare Advantage plans, and commercial healthplans. In cases where we are not a participating provider under your insurance plan, rest assured that we will work closely with your insurance provider to ensure you receive the best possible care. Our goal to ensure pulmonary rehab at home is to make the process seamless and accessible for you.

Pulmonary rehabilitation is covered as a Medicare Part ‘B’ benefit, with Medicare shouldering the cost for qualifying patients. While patients may have a 20% copay (approximately $300 for the entire program) and a Medicare Part B deductible ($240 in 2024), these expenses can be mitigated or eliminated entirely by Medigap insurance coverage. For added flexibility, we offer the option of a payment plan to ensure accessibility and convenience for all with chronic lung conditions.

The program’s standard duration is 12 weeks, offering a comprehensive approach to pulmonary rehabilitation. However, we recognize that each patient is unique, and durations may vary based on individual needs. As the program concludes, our emphasis shifts towards equipping patients for independent continuation of pulmonary rehab at home. In the final weeks, we place a strong focus on preparing individuals to sustain their exercise routines autonomously and uphold the lifestyle improvements achieved throughout the program. We provide guidance and resources to empower our patients in maintaining their progress beyond the structured program duration.

We provide patients enrolled in our virtual pulmonary rehabilitation program with essential tools for their journey. Participants receive a digital pulse oximeter for their use throughout the program as well as exercise bands to support their physical activities. The digital pulse oximeter is a user-friendly Bluetooth device that seamlessly connects to the Morlen Health Convenient Care app on their smartphones, enhancing accessibility and enabling efficient tracking of progress and vital signs.

Patients completing the program can anticipate a comprehensive range of improvements based on their individual needs. Referring providers can stay informed about their patients’ progress through various channels in Morlen Health’s Home-Based Pulmonary Rehabilitation program. Initially, a copy of the patient’s initial intake assessment and customized treatment plan is promptly sent to the referring provider. Throughout the program, our healthcare team conducts assessments of patients’ progress at 30-day intervals and at discharge. To ensure continuous communication, copies of these progress and discharge assessments are routinely shared with referring providers. This transparent and collaborative approach aims to keep all involved parties well-informed about the patient’s journey and achievements.

Morlen Health’s home-based pulmonary rehabilitation program is currently available in the states of Oregon and Washington, and we are certified to provide Telehealth services in Florida. Our home-based pulmonary rehab program is designed to be flexible and accommodating. Participants typically have one visit with their respiratory therapist, one group exercise class, and one educational class each week, with each session lasting about an hour. Additionally, participants are encouraged to incorporate 20 minutes of home exercise into their routine, aiming for 3 to 5 times per week. This well-rounded approach ensures a comprehensive and manageable program that can be seamlessly integrated into participants’ daily lives while promoting respiratory health and overall well-being.
